The waiver applies to Class A or B CDL depending on military vehicle experience.

Must have been licensed to drive a heavy military vehicle for at least two years with a clean driving record. Submit Form DR 2742 (skills test waiver application) and Form DR 2172 (10-year license certification) to CDL Unit, Room 154, 1881 Pierce St, Lakewood, CO 80214.
